Why do I feel my heart beating in my ears and at the base of my neck at night?

Could it be that my pressure medication is not strong enough? I take teva valsartan HCTZ 80+12.5 mg 1 CO in the morning and I take 1co of vitamin D. I take my blood pressure from time to time and it is usually 120/70,130/80,122/80,121/70,pls play from 72 to 76.I have extrasystoles.I put estrogel before going to bed.I have been taking my medication for about 3 to 4 years.No particular pain.
