Hello,
In order to better advise you, I need to validate two elements.
What is his temperature?
How is his general condition?

I also saw that you asked a second question in which you mentioned that the COVID test is done
I guess you are waiting for the result?

Hello yes waiting for the result he is 39 and is very ugly... hardly eats, drinks little

His general condition seems quite affected. At this time, it is important to keep the fever down to avoid dehydration. Have you given acetaminophen or ibuprofen so far?

Yes, the first time he threw it up and I gave it again this morning and tonight only tylenol

It often happens that big fever spikes require a stronger analgesic than Tylenol. I strongly suggest that you give Advil (or Motrin) for tonight and the next few days at a dose every 6 to 8 hours. It is more effective and lasts longer to relieve the fever. In some cases, Tylenol does not reduce the fever enough (especially if the dose used is insufficient)
If you want I can calculate the dose of Advil (and at the same time I can validate the dose of Tylenol). What is his weight?

It weighs 27 lbs

The correct dose of Advil would be 3 ml (122 mg) every 6 to 8 hours (infant formula 40 mg/ml).
The correct dose of Tylenol is 2.3 ml (184 mg) every 4 to 6 hours (infant formula 80 mg/ml). The maximum number of doses per day is 5
It is also possible to give Tylenol and Advil alternately every 3 hours
