Is there a risk of interaction between the EVRA contraceptive patch and some common antibiotics and antacids?

Hello, I have just changed my contraception from the pill to the EVRA patch with the advice of a health professional. I have just read the instructions and I have some questions. I have read that there may be interactions between EVRA and certain medications such as antacids and acetaminophen. I may take Tylenol for a headache or Pepcid for heartburn once or twice a month. Is there a real risk of interaction with these medications? If so, is there a risk to the effectiveness of the contraceptive? Also, something quite interesting: it is written on the leaflet that there can be an interaction with grapefruit juice. The professional who prescribed the patch did not mention this to me. Is the risk really serious if I drink a glass of grapefruit juice? Also, I noticed that the instructions in the leaflets can vary. I would have liked to have confirmation that the patch can be stuck on the upper chest (above the breasts). Thank you in advance for these precisions. Good day to you!

Hello, thank you for your question!
First of all, the manufacturer recommends applying the patch on the buttocks, abdomen, upper arms and back. The manufacturer also recommends the upper torso, but this is rarely seen in practice because it can be visible at that time
The manufacturer also mentions a "hypothetical mechanism" where the use of antacids could decrease the intestinal absorption of progesterone. However, there is no evidence that this has any clinical impact, especially since Evra is not taken by mouth, so intestinal absorption is not relevant.
There is no interaction with antibiotics, with a few exceptions. This will be on a case by case basis and your pharmacist will have to check if necessary
There is no problem with acetaminophen or grapefruit juice either
I hope that I have reassured you and I am available if you have any further questions!

Hello Tom Samaha,
Thank you very much for your quick answer.
I was indeed a bit surprised by all the "possible" risks of interactions mentioned in the leaflet, whereas the professional who prescribed it to me had not mentioned it. But your explanations reassure me!
I have one last question: it is mentioned that the patch is waterproof, so you can easily take showers or baths. But it is recommended not to leave it next to heat sources and to avoid heating pads, saunas and prolonged exposure to the sun. I was wondering how this type of heat would affect the effectiveness of the patch and if I can still take hot showers every day without fear.
Thanks in advance.
Have a great Monday!

Very good question!
Saunas, hot baths and other heat sources can increase the release of the patch, which may, in some cases, increase the risk of side effects. However, these are mild, temporary, and quite rare. It is still recommended to avoid these heat sources to avoid large fluctuations in the release of the patch.
